Job Summary:
The Palliative Care Nurse is responsible for providing specialized nursing care and support to patients facing serious illnesses, focusing on pain and symptom management, emotional support, and improving the quality of life. The nurse collaborates with interdisciplinary teams to develop and implement comprehensive care plans that meet the physical, emotional, and spiritual needs of patients receiving palliative care. Furthermore, the Palliative Care Nurse educates patients, families, and caregivers on palliative care interventions and facilitates effective communication among the healthcare team.
Key Responsibilities:
1. Conduct comprehensive assessments of patients' physical, psychological, social, and spiritual needs to develop individualized palliative care plans.
2. Administer medications and treatments to manage pain and alleviate symptoms while ensuring patient comfort and dignity.
3. Monitor and evaluate patients' responses to treatments, adjusting care plans as necessary to maintain optimal physical and emotional comfort.
4. Collaborate with interdisciplinary teams, including physicians, nurses, social workers, and chaplains, to develop and implement holistic care plans.
5. Provide emotional support and counseling to patients and families, promoting effective coping strategies and addressing their concerns and anxieties.
6. Educate patients, families, and caregivers on palliative care principles, interventions, and available resources.
7. Facilitate open and effective communication among healthcare team members, patients, and families to ensure coordinated and patient-centered care.
8. Document patient assessments, interventions, and outcomes accurately and timely in electronic medical records, adhering to privacy and confidentiality regulations.
9. Participate in continuous professional development activities to enhance knowledge and skills in palliative care nursing.
10. Adhere to legal and ethical standards, professional nursing guidelines, and organizational policies and procedures.
Required Skills and Qualifications:
1. Valid nursing license in [state/province] and [country].
